Work as a Community Nurse Case Manager for a complex care provider, supporting clients to live independently at home and within their wider communities. This community role will be based in Crewe, Cheshire.
The Nurse Case Manager will manage a caseload of clients with complex needs in the community. Responsibilities include:
1. Initial assessments in the community
2. Training and assessing workers' competencies, and allocating support workers to patients
3. Collaborating with the wider multidisciplinary team (MDT)
4. Attending meetings and making referrals
5. Holding team meetings
6. Managing a team of non-clinical case managers
We are seeking an RGN qualified nurse with excellent clinical skills in complex care, including PEG feeds, ventilators, and tracheostomy management.
Applicants must be a car driver, as the role is community-based.
Preference for candidates with Band 6 ICU/ITU nursing experience, ideally with community nursing experience. Community experience is essential unless you have held a senior role (Band 6 or above) in an ICU/ITU setting.
This full-time role is mainly Monday to Friday, 9 am to 5 pm, with some flexibility. The office base is in Crewe, with travel around Crewe, Cheshire, and possibly to other local branches.
The salary is negotiable depending on experience, ranging from £45,000 to £50,000.
Experience with tracheostomy and ventilators is essential.
