We’re looking for a compassionate and proactive individual to join Active North Yorkshire as an Active Health Referral Co-ordinator. In this rewarding role, you’ll be the first point of contact for individuals referred into our musculoskeletal health programmes—helping them take the first steps toward improved wellbeing.
What you’ll be doing:
* Providing triage and personalised support to individuals referred into our musculoskeletal health programmes
* Using motivational interviewing and positive psychology to guide people through behaviour change
* Coordinating appointments and liaising with Health and Wellbeing Coaches to ensure a seamless customer journey
* Promoting services and collaborating with referral partners, including GPs and community organisations
* Recording and reporting on outcomes using digital systems and collecting client feedback
* Supporting clients in identifying personal health goals and developing tailored action plans
* Signposting to other services and ensuring inclusive access for underrepresented group
What we’re looking for:
* Excellent communication and organisational skills
* A friendly, empathetic, and solution-focused approach
* Confidence using IT systems and managing digital admin tasks
* Knowledge of healthy lifestyle changes and behaviour change principles
* Experience in a supportive or health-related role is desirable
* Understanding of MECC (Making Every Contact Count) and motivational interviewing is a plus
