To act as a senior clinicalprofessional within the Virtual Wards.
Virtual wards is a service delivered in collaboration with our local acute trust and local authoritypartner. Working in partnership with the hospitalcase finders and virtual ward consultants, the post holderwillact as an independent autonomous practitioner to provide specialist treatmentand advice to patients and staff which will contribute to the assessment,diagnosis, care and treatment of patients at home preventing unnecessaryhospital admission.
Possess and demonstrateextended clinical skills in the assessment, diagnosis, treatment and managementof a wide range of patient care needs. Professional autonomy and clinicaljudgement will be exercised in managing, referring and signposting patients onto wider community services and assets.

Clinical
Participate in and undertake triage to ensuresmooth progress of patients through the system.
Comprehensivelyassess patients with complex physical, mental and social care needs usingadvanced clinical examination skills, detailed history taking and biometricmonitoring and evaluation skills.
Receive patients withundifferentiated and undiagnosed health care problems and initiate and conductclinical investigations as appropriate.
Analysesigns and symptoms, laboratory tests and other measures of function, informulating a diagnosis.
Perform amedication review and prescribe and change medications as required inaccordance with national and local guidance.
