Overview

If you are a doctor who is looking for something a bit different, enjoys mentoring, education and working in a multi-disciplinary team then we would love you to apply to join our small team of Community Specialist GPs and Practice Educators. We’re looking for experienced GPs to provide clinical support, leadership and education in a number of different settings across Derbyshire. Clinical support is needed for community inpatient rehabilitation wards and for Urgent Treatment Centres, in addition to educational sessions to support trainees to enhance and develop their practice in areas such as non-medical prescribing, clinical decision making and clinical reasoning.

If you have experience of supporting trainees in advancing practice roles, medical students or specialty trainees, a special interest in caring for patients with urgent or complex needs, including frailty, and/or leading clinical services/teams then we would encourage you to apply. We have opportunities across Derbyshire, delivering care and supporting trainees in community hospitals, primary care, urgent treatment centres, community teams and specialist clinics.

Up to 20 hours are available on a 12-month fixed term contract; these can be shared by multiple successful applicants to fit in with other commitments. Applicants will need to be willing to work flexibly and to travel. Geographical coverage can be discussed and negotiated within reason and an element of remote and virtual working is possible.

To work as part of a small team of Community Specialist GPs / GP Practice Educators undertaking clinical and educational responsibilities within DCHS.

Clinically this role will include covering some weekly ward rounds for our Community Hospital wards, providing prescribing support to non-prescribing clinicians and advice and guidance to clinical practitioners supporting DCHS services including pathway 2 beds.

The role also incorporates Practice Educator sessions. These sessions support continuing professional development and training for the range of clinicians working at, and towards, a level of advanced clinical practice as described by NHS England. This will include clinical practitioners working in the four Urgent Treatment Centres, the wards in DCHS community hospitals, community teams and primary care.

Successful candidates will also be expected to provide an element of clinical leadership to agreed activities, aligned as far as possible to individual skills and interests.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

The Multi-Professional Practice Development Team, which this role is part of, supports and promotes Advanced and Enhanced Practice training and those clinicians working on our inpatient wards.

Although distributed in geography and professional backgrounds, the Multi-Professional Practice Development Team are a tight knit and supportive team of around 30 colleagues. There will be direct work with patients and their carers, either in person or remotely within the clinical element of the role. You will also support the clinical practice and development of DCHS clinicians and so supervision, mentoring, support and education are an important element of the role. This may be in providing 1-1 support in an educational GP surgery, shadowing a trainee during their usual duties, undertaking Workplace Based Assessments (WPBAs) or reviewing patients together in a community ward, Urgent Treatment Centre, clinic or care home setting. As a medically trained practice educator the post-holder may be asked to provide medical supervision/input for trainees undertaking non-medical prescribing and clinical assessment training modules and as part of this may be asked to take the role of Practice Supervisor or Clinical Mentor for particular trainees. Appropriate training and support for undertaking these roles will be provided as required.

In addition to 1-1 support for clinicians, post-holders may also be asked to facilitate small group ‘learning from practice’ groups to facilitate learning from and reflection on practice and other small group teaching groups around clinical skills and examination development. There is also the opportunity for larger group teaching sessions, currently delivered remotely, to be developed and delivered on topics of specific interest and expertise, contributing to a rolling programme of clinical education on clinical practice and non-medical prescribing.

Leadership which supports the organisation in clinical, professional and operational activities will form a part of this role. Leadership responsibilities are shared across team members and aligned to individual interest and skills.

It is expected that the team will mostly be self-managing whilst supported by the Clinical Directors for Multi-Professional Development and for Multi-Professional Community Practice, who oversee job planning, workforce development and appraisal for the team. It is also hoped that the Community Specialist GPs / Practice Educators will participate in the development of a learning network of Practice Educators and Community Clinicians working across Derbyshire.
