Permanent, Part Time 18.5 hours per week.
Location: Based on-site predominantly at the Bristol Royal Infirmary, with periodic coverage at alternative bases.

In this Coordinator role, as part of the Discharge to Assess, Triage and Response Team, you will work with service users, families, and carers currently in hospital. Your role is to support their safe and appropriate discharge, understanding their unique circumstances and priorities. You will champion the principles of choice and control, supporting individuals to make their own decisions and be empowered to organize services and manage risks in ways that suit them.
You will support people and their families or carers upon hospital discharge, ensuring they receive relevant aftercare and support, directly impacting their lives.
You should be highly motivated, proactive, and adaptable, able to respond effectively to diverse individuals and collaborate with external and internal partners. Building relationships and working in partnership will be key strengths.
Your responsibilities include maintaining and updating information systems and databases to ensure accurate service transactions. You will advise customers, contacts, external organizations, and colleagues on policies, procedures, and processes, maintaining effective working relationships.
