A wonderful surgery in the Twickenham area of London are seeking an experienced Practice Nurse.We are a long established, teaching practice with over 8000 patients. We pride ourselves in excellent patient care and are seeking a hardworking, enthusiastic Practice Nurse with experience and training in long-term conditions to join our expanding practice team. This is a permanent post.

16hours per week. 8.30am until 5.30pm on Tuesdays and Thursdays. Hours and day may be negotiable for the right candidate.

The post-holder will implement and lead on a full range of promotingand managing their own and others health and safety and infection control asdefined in the practice Health & Safety policy, the practice Health &Safety manual, and the practice Our practice makes use of clinical system EMIS.

Infection Control policy and publishedprocedures.


Main duties of the job

The idealcandidate will be an experienced Practice Nurse who is pro-active in thedevelopment of nurse competencies to meet practice needs. Desirableskills/experience includes but is not limited to, childhood immunisations, adultimmunisations, travel vaccinations, cytology, contraception, chronic diseasemanagement, spirometry, ECG, phlebotomy, annual diabetic / asthma checks, QOFrelated admin etc.

Job responsibilities

Job Summary

The post holder is anexperienced nurse, who is responsible for ensuring the delivery of safe andeffective patient care to the practice population using autonomous clinicalskills and a broad in-depth theoretical knowledge base. The post holder willmanage a diverse clinical case load, dealing with the presenting patientsneeds within a primary care setting. They will contribute to developing thenursing services offered by the practice. The practice nurse worksindependently and contributes to audit, research and supporting, implementingand evaluating local standards, policies, guidelines and care pathways. NMCrequirements will be met and the individual will work within the NMC Code ofConduct to agreed practice standards and protocols.

Thepost holder is accountable for his/her own practice and will work independentlymanaging their own clinical workload responding effectively to patient needsand ensuring patient choice and ease of access to services.

Tomentor and support other nurses and staff in developing and maintainingclinical skills.

Toparticipate in clinical supervision.

Tohave an awareness of budgetary constraints and help make cost effectivedecisions.

Touse referral pathways for nurses as appropriate.

To provide input and advice on evidencebased practice and care interventions within their sphere of knowledge.

Contributeto the practice achieving its GMS contract obligations including quality,organisational and access targets.

Participatein clinical supervision with an identified senior nurse.

Maintainaccurate records according to NMC rules.

Ensureall data protection requirements are met when gathering, recording and storingpatient data.

Liaise and maintain goodworking relationships with all members of the practice team. Work within themulti-disciplinary team within the practice and across the wider health system.

Primary Duties & Areas of Responsibility

Clinical Role

Assess,plan, develop, implement and evaluate treatment programmes for patients thatpromote health and well-being.

Helpthe practice develop and deliver specialist primary nurse led services,including chronic disease management (including for all areas of QOF). Managesprogrammes of care for patients with chronic diseases by planning, providingand evaluating care under agreed guidelines and patient group directions.Maintains disease registers in liaison with administrative staff. Undertakesannual reviews of patients understanding and ability to self manage.

Makeprofessionally autonomous decisions for which he/she is accountable and providesafe, evidence based, cost effective, individualised patient care.

Wherenecessary evaluate patient test results in conjunction with the patient andtheir GP and ensure the appropriate course of treatment is initiated.

Makeappropriate referrals to other members of the primary, community and secondarycare teams.

Undertakeregular medication reviews for patients for groups of patients deemed suitableby the clinical team.

Providecomprehensive travel health advice for patients prior to travel includingmalaria prophylaxis, safe sex, sun protection, food hygiene, first aid andemergency medication, health insurance and vaccinations.

Administerchild and adult immunisations and vaccinations in accordance with national andlocal programmes. Competent in anaphylaxis and resuscitation techniques.Ensuring safe storage, rotation and disposals of vaccines and drugs and thatadequate stock levels are maintained

Undertakeprocedures including: venepuncture, glucose tolerance tests, BP management,urinalysis, peak flow, spirometry, new patient health checks, pregnancytesting, stitch/clip removal, ear syringing, cytology, wound management andcryosurgery.

Providesupport and chaperoning to the doctors for minor surgery and other procedures.

Promotehealth and well being, giving general education advice on diet, contraception,smoking chronic disease management, exercise etc...

Initiateand carry out programmes of health screening as agreed with the practiceclinical team.

Enable,support and encourage individuals, families and groups to address issues whichaffect their health and social well being.

Teaching & Mentoring Role

Promotea learning environment for patients, nurses and other health professionals.

Assist with the planning andimplementation and teaching for practice staff including medical students,nurse students, nurses and health care assistants.

Professional Role

Attendand contribute to relevant internal and external meetings as required includingpractice clinical meetings, nurse team meetings and gold standard frameworkmeetings.

Maintaina personal progress and training plan to keep up-to-date with current thinkingand developments as well as identifying own training deficiencies and attendingsuitable courses and appropriate.

Promoteevidence based practice through use of the latest research based guidelines.

Monitorthe effectiveness or their own clinical practice through quality assurancestrategies such as the use of peer audit and review.

Maintainprofessional registration.

Workwithin the latest NMC Code of Professional Conduct.

Recordaccurate consultation data in patients records in accordance with NMC guidanceand other standards.

Keepup to date with pertinent health policy and work with the practice team toconsider the impact and strategies for implementation.

Workcollaboratively with colleagues both internally and externally.

Demonstrateleadership.

Pro-actively promote the roleof specialist nursing expertise within the practice and with the public.

Managerial Role

Identifyopportunities to delegate tasks to junior members of the nursing team.

Helpthe practice develop in a cost effective manner, monitoring and using resourcesappropriately e.g. stock control of dressings, vaccinations.

Participatein practice meetings as required.

Participatein audits and inspections as required.

Aspart of the nursing team ensure all appropriate practice policies are fullyimplemented.

Promoteseffective communication, relationships and team working within the practice.

Providesclinical educational advice to support develop of the team and range of servicesprovided.

Partakesin clinical supervision for own development purposes.

Facilitates and promotes theuse of up to date technology to develop practice.
