About the Role
An established community-based service is seeking a compassionate and skilled Family Support Practitioner to provide targeted support to children, families, and individuals experiencing vulnerability, domestic abuse, and complex social challenges.
The role focuses on early intervention, safeguarding, and practical family support, working collaboratively with partner agencies to improve outcomes and promote long-term stability, safety, and wellbeing.
Key Responsibilities
Family Support & Casework
* Manage a caseload of children and families requiring early help and targeted intervention
* Deliver one-to-one practical and emotional support to parents, carers, and families
* Complete needs assessments, risk assessments, and support plans, reviewing progress regularly
* Support families to access housing, benefits, education, health, and community-based services
* Promote resilience, independence, and positive outcomes for children and families
