Overview

As a Recovery Coordinator with Change Grow Live you’ll work with people who use our services, helping them get to where they want to be. You will work as part of the Switch Team, based within Hull Renew. The team supports people with their or their loved ones’ drug or alcohol issues.
You will work collaboratively with partnership agencies to support people who have been placed in accommodation (supported lettings) by the local authority. To deliver coordinated and intensive targeted support to this cohort to reduce tenancy breakdown, a Multi-Disciplinary Team (MDT) approach will be adopted, including input from specialist alcohol & drug provision, tenancy sustainment support and other disciplines as appropriate to each individual’s assessed needs. You will manage a diverse caseload and deliver intensive drug/alcohol support, working holistically with other agencies involved in supporting the person.

Full Time Hours 37.5 per week
Full Time Salary Range £27,861.26 - £32,002.35 (based on full time hours, pro rata for part time)
Please note Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
About You
* Work as part of multi-disciplinary team to deliver high-quality support to rough sleepers and those at risk of homelessness.
* Arrange and attend professional meetings with services in relation to the people we support, e.g. MARM meetings.
* Foster a compassionate and inclusive environment where every person is treated as an individual, with flexibility, curiosity, and active listening central to our approach.
* Collaborate with your team and safeguarding experts to identify and mitigate risks for people we are supporting.
* Coordinate and lead a proactive solution-focused approach for people, ensuring they can access appropriate treatment options.
* Provide personalised assessments and develop and regularly review holistic recovery and support plans with the person and other professionals to reduce barriers to recovery and meet their needs.
Experience and qualifications
* Experience of working within housing or homelessness services.
* Working knowledge of mental health, substance misuse and complex needs.
* Knowledge of homelessness, housing, benefits, and welfare.
* Knowledge of homelessness law.
* Working towards a professional housing / homelessness qualification.
* Experience working with vulnerable groups in a professional, personal, or voluntary capacity.
