We are currently seeking an experienced full-time Care Coordinator to join our client organisation, Chosen Care Group.
Our goal is to deliver high-quality care, ensure optimal standards in communities, prevent illness, reduce hospital visits, and improve management of chronic conditions.
Role of a Care Coordinator
The Care Coordinator acts as a vital link between care workers, clients, Next of Kin (NOK), and professional counterparts. If you are a bright, bubbly individual capable of multitasking and providing excellent customer service, this role could be ideal for you.
Requirements
* Previous Care Coordination experience
* Excellent customer service skills
* Experience with planning software such as People Planner, CM2000, etc.
* Ability to work under pressure with good time management and initiative
* Strong influencing skills to gain respect and convince others
* Excellent communication skills, including telephone manner and relationship building
* Willingness to participate in an on-call system for out-of-office hours
* Good knowledge of Microsoft Office packages

Main Duties
* Manage daily care service operations, ensuring high standards of care
* Develop staff rotas for patient care, activities, and emergencies
* Monitor compliance with regulatory standards and policies
* Collaborate with patients and families to develop personalized care plans
* Train and supervise staff to ensure quality care
* Oversee medication administration with proper documentation
* Facilitate access to external support services and healthcare professionals
