Looking for something different from traditional general practice? We are seeking a highly motivated and committed General Practitioner to join our clinical team within Combined Healthcare. This is a pivotal post in delivering quality and innovative primary care services. The team is friendly, well organised, and forward thinking, part of an outstanding CQC organisation and HSJ Trust of the Year.
Commitment: 6 sessions per week (plus CPD), to be undertaken on Wednesdays, Thursdays and Fridays, primarily based at Moorcroft Medical Centre/Moss Green Surgery, with cross cover to support other bases within our integrated practice portfolio.
We are a high-achieving, patient-centred practice with a care model to improve patient experience, providing urgent and routine appointments. Standard clinical sessions and administration time for non-patient facing work within the session are routinely included. You will have excellent clinical support from our extended clinical team, including ANPs and UCPs. Weekly protected learning events support CPD, with the benefits of being part of a larger Trust.
You will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients. Teamwork is essential, working as part of our senior clinical team to provide quality care and to support the wider practice core clinical workload. Responsibilities
* In accordance with the Job Plan as agreed, undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions, and dealing with queries, paperwork and correspondence in a timely fashion.
* Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
* Assess the health care needs of patients with undifferentiated and undiagnosed problems.
* Screen patients for disease risk factors and early signs of illness.
* In consultation with patients and in line with current Practice disease management protocols, develop care plans for health.
* Provide counselling and health education.
* Admit or discharge patients to and from the caseload and refer to other care providers as appropriate.
* Record clear and contemporaneous consultation notes to agreed standards using the practice electronic clinical system.
* Collect data for audit purposes.
* Compile and issue computer-generated acute and repeat prescriptions.
* Prescribe in accordance with the Practice prescribing formulary (or generically) where clinically appropriate.
* Undertake the normal duties and responsibilities associated with a GP working within primary care.
