Overview
Integrated Care Clinician (Nurse) – Swansea. The post holder will undertake telephone triage assessments of patients in an efficient way. They will advise the most appropriate intervention to meet the needs of the patient using best clinical practice, adhering to Trust Policy, protocols and guidelines. They will work in an autonomous capacity, assessing and advising callers within agreed guidelines. NHS 111 Wales provides urgent out of hours care, 24-hour health information with sign posting and access to urgent dental services.
As a Band 6 Triage and Assessment nurse, you’ll have the autonomy to make decisions but be supported by our experienced team of senior clinicians, practice coaches and multi-disciplinary colleagues.

Responsibilities
* Carry out telephone triage assessments of patients and advise the most appropriate intervention in line with clinical guidelines and Trust policy.
* Work autonomously within agreed guidelines, using clinical assessment skills to advise callers.
* Collaborate with urgent care GPs, Pharmacists and Mental Health Practitioners as part of a multi-disciplinary team.
* Operate in a remote clinical decision-making environment to manage a broad spectrum of patients, from minor ailments to urgent conditions.
* Contribute to delivery of a clinically effective, patient-centred urgent care service across Wales.
* Apply professional standards and participate in continuous professional development and revalidation as required.
Qualifications and experience
* Registered Nurse (Band 6) with relevant clinical experience in triage, assessment or urgent care settings.
* Ability to make autonomous clinical decisions with appropriate supervision and access to senior support.
* Strong communication skills and ability to work in a remote decision-making environment.

In line with the Trust’s Starting Salary Procedure, all applicants will start at the bottom of the band for the position applied for but can apply for a higher salary if they have previous experience relevant to the position.
