Overview
We are seeking a qualified and experienced Social Worker to join our County Wide Hospital Discharge Team. This temporary post (12 months) covers the Chesterfield Hospital Discharge Team and offers an exciting opportunity to deliver high‑quality care within an MDT setting.
Responsibilities
* Provide short‑term assessment and support to maximise independence and prevent unnecessary admissions.
* Work within tight discharge timescales to enable patients to continue living in their own homes as long as possible.
* Collaborate with a multidisciplinary team in a busy integrated setting.
* Support front‑door processes and admission avoidance to keep individuals safely in the community.
