What you will do
* Provide comprehensive and compassionate mental health care to individuals in the community engaged with the Home Treatment Team.
* Conduct assessments, devise treatment plans, and deliver therapeutic interventions.
* Work collaboratively with a multidisciplinary team to ensure holistic care is provided.
* Maintain detailed and accurate patient records and promote good practices.
* Engage with patients, families, and carers to develop effective support networks and educational resources.
