The post is based at Falmouth Community Hospital Bungalow, Falmouth, with home visits to people in the community and care homes. The role is Band 6, in the Dementia and Older People’s Mental Health services. Contract: Permanent; Hours: Full time - 37.5 hours per week; Working pattern: Monday to Friday 09:00 – 17:00 (excluding Bank Holidays). Salary: £38,682 - £46,580 per annum. Job ref: 201-25-660.

Responsibilities
* Provide assessment, treatment and care co‑ordination to two client groups: (1) People living with dementia; (2) Frail adults living with a range of mental health needs with age-related physical health issues.
* Work as a senior member of the team and liaise with a wide range of statutory and non-statutory agencies to develop and deliver personalised care under the Care Programme Approach.
* Manage a caseload under CPA, placing the person at the centre of care in a busy but rewarding role.
* Work in partnership with carers/family members and community services.
* Visit people at home and in care homes as part of the Fal DOPMH Team; support community health and social care teams.
Qualifications and Experience
* Current professional registration (NMC or relevant Allied Health Professional Registration).
* Professional qualification RMN, RGN or Allied Health Professional.
* Postgraduate qualification or equivalent experience in a psychological therapy (e.g., CBT).
* Substantial experience of managing a busy community caseload with dementia and/or mental health problems and frailty due to ageing.
* Experience of providing clinical supervision and working in a multi-disciplinary and multi-agency environment.
