Join Forward Trust as a Health and Wellbeing Practitioner at HMP Aylesbury
Be part of a team that changes lives through substance misuse support and psychological care. This role offers the opportunity to work directly with individuals in custody, supporting their journey towards recovery, rehabilitation, and reintegration.
Working within an integrated healthcare framework, you'll support adult men in prison (18+) with drug and/or alcohol misuse issues. You will deliver a range of psychosocial interventions, combining harm reduction and abstinence-based approaches, tailored to each individual.
Key Responsibilities
1. Managing a caseload, conducting assessments, and care planning.
2. Delivering structured 1:1 and group sessions, including accredited treatment programmes.
3. Providing harm reduction advice and relapse prevention support.
4. Supporting throughcare planning in collaboration with partner services such as Probation, Housing, Employment, Healthcare, Gym, and transitional teams to prepare individuals for release and community reintegration.
5. Occasionally acting as a second signatory or conducting oral swab testing to complement clinical services.

We seek someone passionate about empowering others with a solid understanding of substance misuse interventions in a secure setting.
Essential Experience and Skills
* Experience delivering psychosocial support and structured interventions.
* Skilled in care planning, assessment, and caseload management.
* Knowledge of harm reduction, abstinence, and motivational techniques.
* Strong team-working abilities and experience of multi-agency collaboration.
* Confident communicator with excellent record-keeping and IT skills.
