Serve as a Care Coordinator under the Care Programme Approach (CPA), crafting and reviewing recovery-focused care plans with users and carers. Deliver evidence-based nursing interventions, psychosocial support, and monitor medication compliance. Assist in relapse prevention and crisis planning to foster a secure recovery environment. Appraise and manage risks associated with self-harm, suicide, and safeguarding concerns. Collaborate regularly with a multidisciplinary team, including psychiatrists, psychologists, social workers, and support staff. Participate in team meetings, contributing to case reviews and discussions. Support effective medication management, monitoring effects and administering depot medication when necessary.
