Hours: Monday - Friday, 8:30 AM - 4:30 PM

Rate: £20-£23/hr PAYE DE
Client Brief:
A well-regarded NHS service in South Wales providing a broad range of community and hospital-based healthcare. The team delivers person-centred rehabilitation and therapy services, supporting individuals in regaining independence and reducing the need for long-term care. With a focus on community integration and proactive intervention, the service operates in collaboration with multidisciplinary professionals to ensure high standards of care across the region.
The Role:
They are seeking a dedicated and experienced Band 6 Occupational Therapist to join their team on a locum basis. The role involves delivering high-quality assessments and interventions within patients' homes, supporting individuals to maximise independence and reduce reliance on long-term care services.
Key Responsibilities:
* Conduct person-centered assessments and provide tailored interventions in patients' homes
* Support functional independence in daily activities and reduce dependence on formal care
* Develop and implement rehabilitation and discharge plans
* Liaise effectively with families, carers, and professionals across services
* Maintain accurate, timely clinical documentation
* Work independently while supporting team objectives
Requirements:
* HCPC registered Occupational Therapist
* Previous experience in community-based or rehabilitation OT roles
* Access to own vehicle and ability to travel for home visits
* Training in Manual Handling, Safeguarding, and related competencies
* Excellent clinical reasoning, organisational, and communication skills
Desirable:
* Knowledge of home adaptations, equipment provision, and community support services
* Experience in reducing care packages and promoting patient autonomy
