Buckinghamshire Healthcare NHS Trust provide a wide range of high quality acute and community services from two acute hospital sites in Stoke Mandeville and Wycombe, and community hospitals at Amersham, Thame, Marlow, Buckingham, Chalfonts and Gerrards Cross. More than 6,000 staff serve residents across Buckinghamshire, Thame (Oxfordshire), Tring (Hertfordshire) and Leighton Buzzard (Bedfordshire) - a combined population of 500,000. It serves a much larger population, 1.5m for burns and plastic services and 14m for spinal injuries.

Your main duties include:
* Comprehensive specialist assessment, review and evaluation of the needs of patients and carers within a 0-2 hour, 2-24 hour timeframe
* Being responsible for undertaking an urgent community response to people in their own homes to prevent hospital conveyances and keeping patient safe at home with the appropriate clinical interventions.
* Working with a range of professionals in order to ensure care is appropriate and delivered in a timely manner.
* Demonstrating professional skills and competence in leadership, care planning / co-ordination, and care and treatment delivery. Support the development of clinical pathways

* Experience of working in a community/in-patient setting for a minimum of 2 years post registration
