We are looking for a dedicated Homelessness Outreach Worker to join a unique team which delivers vital support to individuals sleeping rough or at risk of homelessness. This is a meaningful and fast-paced role where every day you will make a visible difference to people’s lives. About the Role As a Homelessness Outreach Worker, you will: Provide street-based outreach to rough sleepers across the community. Build trusting and compassionate relationships with people facing homelessness. Carry out assessments and create individualised support plans based on each person’s needs. Support individuals to access emergency accommodation, healthcare, benefits, substance misuse services, and long-term housing. Work closely with local authorities, housing partners, health teams, and specialist agencies. Promote safety, wellbeing, and safeguarding at all times. Keep accurate case notes, risk assessments, and monitoring data. This role is varied, rewarding, and essential in supporting people to rebuild their lives. Experience we’re looking for: Empathetic, resilient, and passionate about supporting vulnerable people. Able to work independently in the community and manage a diverse caseload. Confident in building relationships and engaging people with complex needs, including mental health issues, trauma, or substance misuse. Comfortable working with partner agencies and advocating on behalf of clients. Organised, proactive, and calm under pressure. Knowledgeable about homelessness services Why apply?
