To work as a member of the multidisciplinary care team, providing and managing holistic care to people who have life limiting conditions. Based in our Fair Havens hospice, overlooking beautiful Priory Park, you will support our Adult In-Patient team, caring and supporting our patients. The shift patterns for this role is three long shifts per week, and there is a requirement to work day, night, weekend and bank holiday shifts, as per rostering requirements. Responsible for assessing and managing the needs of patients, undertaking the assessment, planning, implementation, and evaluation of care, following clearly defined policies, protocols, procedures, or codes of conduct. Our specialist Care Teams can support them and their family living throughout illness, death and bereavement in the comfort of their own home and through our hospice services, Fair Havens and Little Havens.
