Connect2Halton is currently recruiting for an experienced Qualified Social Worker to join the Short Break Service for Children with Disabilities Team within Halton Council. We are looking to appoint a specialist Social Worker who shares our commitment to provide services that enable families of children with disabilities to have an improved quality of life within their communities. You will have a key role in assessing the needs of disabled children who have complex needs, producing a robust, person centred, Child in Need plan and supporting them to access short break services. Key Responsibilities: Undertake complex assessments for children and young people with disabilities, identifying needs and risks, and determining appropriate short break interventions. Lead and manage complex care planning for disabled children and their families, ensuring packages are tailored to individual needs and promote safety, inclusion, and positive outcomes. Coordinate and oversee joint-funded care packages, working collaboratively with health, education, and :other partners to ensure integrated support. Provide professional leadership within the team, supporting colleagues through mentoring, reflective supervision, and promoting evidence-informed practice.
