Nursing Assistant Practitioner

Closing date is 16 June 2026.

We are a multidisciplinary team of Specialist Nursing, Physiotherapists, Occupational Therapists and Pharmacy working closely to assess and promote wellness and independence of the population of Chorley, Preston and South Ribble presenting with frailty symptoms.

Your role will support the Occupational Therapist and Physiotherapists within this team including undertaking assessments and carrying out treatment plans that fall within the scope of your skills and knowledge under the direction of the therapists.

We are looking for a flexible candidate able to work across a range of settings and with experience supporting Occupational Therapy and Physiotherapy interventions in a community setting, including assessment and provision of equipment according to patient needs.

The role involves working in patients' own homes or with patients in a clinic environment. You will need the ability to travel for home visits.

We seek a Technical Instructor/Assistant Practitioner with excellent communication skills, a natural problem solver and the ability to work autonomously as well as part of a team.


Main duties of the job

* Work as an Assistant Practitioner within the Community Frailty Service.
* Respond to referrals in the community for patients with long‑term conditions who have complex rehabilitation needs.
* Assess and treat patients/clients individually and as part of a multi‑disciplinary team in community settings.
* Use knowledge to assist in the development of links with other community services.
* Contribute to service development, clinical audit and research and development in community therapy assessment and intervention, with a particular emphasis on high‑risk clients who would benefit from functional rehabilitation assessment (e.g., falls, decompensating frailty, advice around optimising occupation and independence).

Person Specification


Education/Qualifications

* Assistant Practitioner Qualification or equivalent.
* NVQ level 3 or equivalent.
* Evidence of continued professional development.


Knowledge

* Knowledge of frailty/long‑term conditions.
* Knowledge of mental illness and disability.
* Knowledge of the cultures and religions of local communities and the ability to work across those cultures.


Experience

* Experience of rehabilitation in the community.
* Ability to work with minimum supervision.
* Ability to organise own time and workload.


Skills and Abilities

* Proven problem‑solving and clinical reasoning skills.
* Ability to work independently.
* Good written and verbal communication skills.
* Ability to adapt communication style to suit different people, including verbal and non‑verbal communication.
* Physical capability to carry out duties, including manual handling of equipment or household items.
