Job Summary
The post holder will work as a Band 6 Community Nurse within a community-based Dementia Assessment & Diagnosis Service, delivering specialist assessments and supporting timely diagnosis and care planning for individuals presenting with cognitive impairment and dementia-related conditions. The role requires autonomous working, effective multidisciplinary collaboration, and clear communication with patients, families, and partner agencies.
Key Responsibilities
Clinical
Undertake comprehensive assessments for individuals referred with suspected dementia or cognitive impairment
Contribute to dementia diagnostic pathways, including cognitive screening and formulation
Complete risk assessments and develop appropriate care and management plans
Monitor mental state, physical health, and functional ability in the community
Provide clear, sensitive feedback to patients and families regarding assessment outcomes
Support post-diagnostic planning and signposting to relevant services
Maintain accurate and timely clinical documentation
Identify and escalate safeguarding concerns appropriately
Multidisciplinary Working
Work collaboratively with multidisciplinary colleagues across health and social care
Liaise with GPs, memory services, and community partners
Participate in MDT meetings and case discussions
Professional Practice
Work autonomously in line with organisational policies, NICE guidance, and professional standards
Practise in accordance with the NMC Code of Professional Conduct
Contribute to service development, audit, and quality improvement activity
Support junior staff where appropriate
Essential Criteria
Qualifications & Registration
Registered Nurse (Mental Health, Adult, or Learning Disability)
Current professional registration
Experience
Experience working with older adults and/or dementia services
Community-based clinical experience
Experience undertaking assessments and care planning
Ability to manage complex presentations and clinical risk independently
Skills & Knowledge
Strong assessment and clinical decision-making skills
Excellent communication skills
Knowledge of dementia pathways and safeguarding processes
Ability to work independently in the community
Competent IT and documentation skills
Additional Requirements
Full UK driving licence
Access to a car for community visits
Desirable Criteria
Previous experience within a memory service or dementia assessment pathway
Experience working within NHS community services
