As a Prison Services Clinical Pharmacist, you will act as the GP Pharmacist for HMP Channing Wood. Your responsibilities include providing medicines optimization services, supporting GPs and dispensary teams on-site, and working closely with patients to address medicine queries and concerns. You will run medication review clinics, utilize SystemOne to identify high-risk patient cohorts, and work with these patients to manage medicine-related risks. You will also implement improvements for long-term conditions, such as de-prescribing and LTC clinics.
You will provide leadership to the wider healthcare team, ensure practice compliance, develop and manage new services aligned with new medicines or NICE guidelines, and collaborate with the Specialist Prison Services Pharmacist to develop and enforce the formulary. Additionally, you will coordinate workload, mentor junior team members, analyze medicines data, and lead prescribing improvement initiatives.
