Job overview
We are looking to recruit a full-time Advanced Clinical Practitioner for Specialist Palliative Care.
This exciting new role will clinically lead our integrated specialist palliative care team.
Working within the four pillars of advanced practice, we require an ACP to support shaping the future of palliative care in West Cheshire, for patients, families, and community teams delivering end of life care daily.
We seek an experienced practitioner who will bring advanced clinical expertise and an evidence-based approach to lead improved integrated community-based specialist palliative care.
We offer a relocation package of up to £8,000
This package is for those relocating to take up employment with Cheshire and Wirral Partnership.
To be eligible, you must live more than 40 miles from the post’s base and relocate within 15 miles of it.

Main duties of the job
This role provides team leadership in clinical workforce development and embedding palliative care competencies across community services. The ACP will develop clinical skills, facilitate supervision, support autonomy, and make advanced clinical decisions. Key objectives include expanding non-medical prescribing, empowering clinicians and the workforce to manage symptoms effectively. The role champions patient safety, continuous improvement, and collaboration with partners.
With support from the team and wider care group, the ACP will focus on quality, research, and innovation, including clinical audit, service evaluation, and quality improvement initiatives.
