We are working closely alongside a Local Authority in Walsallto assist with the appointment of a Social care Facilitatorto work in a residential care setting.
We have both regular shifts (37.00 hours per week) and bank shifts available. Please note that we don't offer sponsorship, and the worker should be able to work full time without restrictions. We would need 5 years work references checked out before you start the shifts.
Rate of Pay: £12.21 - £15.09 per hour
Summary:
The Social Care Facilitator will provide high-quality, person-centred support to individuals accessing Intermediate Care Services in Walsall. The role will focus on promoting independence, supporting hospital discharge, and preventing unnecessary admissions by coordinating care, liaising with multi-disciplinary teams, and ensuring individuals receive the right support at the right time. The postholder will work closely with service users, families, carers, and professionals to assess needs, identify solutions, and implement support plans that enable positive outcomes.
Responsibilities:
* Support individuals through the hospital discharge process and help prevent readmission by arranging timely support and interventions.
* Monitor and review care and support packages to ensure they remain effective and responsive.
* Maintain accurate case records, documentation, and data entry in line with council procedures and confidentiality requirements.
* Contribute to service improvement and development by sharing good practice and feedback from service users.
Qualifications:
* Educated to GCSE level (or equivalent) with good literacy and numeracy skills.
* NVQ Level 2/3 in Health and Social Care
* Evidence of continued professional development in social care or related fields.
* Experience of working in a health, social care, or community support setting.
* Direct experience of supporting vulnerable adults, older people, or individuals with care needs.
* Familiarity with safeguarding policies and procedures.
