Halton 0-19 team are looking for passionate and skilled School Nurses (Specialist Community Public Health Nurses) to join our dedicated and forward-thinking 019 team.
As a School Nurse, you will play a vital role in improving the health and wellbeing of children, young people, and families. You will lead the delivery of the Healthy Child Programme (519), working in partnership with schools, families, and other agencies to identify health needs early, reduce health inequalities, and safeguard vulnerable children. You will have the opportunity to work as part of a supportive, multi-disciplinary team, contributing to innovative approaches in public health nursing, digital health promotion, and targeted interventions.

Main duties of the job
1. The named School Nurse will be responsible for co-ordination of the School Health Plan with their named schools on an annual basis.
2. Contribute to an integrated Public Health Nursing Service by linking to education settings and other partner agencies.
3. Lead on the recommended health assessment and reviews, and supporting mid-teens reviews during transitions as required by national and local specifications.
4. Undertake comprehensive health assessments for identified children and young people.
5. Contribute to the delivery of the national immunisation programme for school-aged children.
6. Perform home visits to children and families advising on health as appropriate.
7. Work in partnership with other key stakeholders to safeguard and protect children and young people.
8. Organise, facilitate and participate in health promotional and health educational activities in schools and community settings, in partnership with school staff and other agencies.
9. Ensure young people not in employment, education or training, or children educated at home receive the universal offer.

Job responsibilities
1. The named School Nurse will be responsible for co-ordination of the School Health Plan with their named schools on an annual basis.
2. Contribute to an integrated Public Health Nursing Service by linking to education settings and other partner agencies.
3. Lead on the recommended health assessment and reviews including reception/year 1 (age 4-5) entry assessment and Year 6/7 (age 10-12) transition assessments.
4. Supporting mid-teens reviews, when young people are embarking on the next transition stage as required by national and local specifications.
5. Lead and coordinate a core programme of interventions according to agreed procedures, ensuring that Trust targets are met.
6. Undertake comprehensive health assessments for identified children and young people, including annual health assessments for those in local authority care in line with the Healthy Child Programme 4-19 years of age.
7. Contribute to the delivery of the national immunisation programme for school-aged children.
8. Plan, implement and evaluate interventions to improve health outcomes; educate children, young people and families to enable them to manage health needs.
9. Perform home visits to children and families advising on all aspects of health as appropriate.
10. Work in partnership with other key stakeholders to safeguard and protect children and young people.
11. Contribute to regular team meetings and multi-agency groups.
12. Support children and young people with identified health needs or those in the child protection system, providing therapeutic public health interventions for the child and family.
13. Provide training to young people, parents, teaching personnel and other professionals where necessary.
14. Provide signposting to other services, referrals and liaison as appropriate.
15. Take and act upon referrals from other agencies and services as appropriate.
16. Provide a confidential support and advice service for young people within a high school or other venue on a regular basis.
17. Organise, facilitate and participate in health promotion and health education activities in schools and community settings, in partnership with school staff and other agencies.
18. Promote healthy lifestyles, positive mental health and the public health agenda.
19. Ensure children not in employment, education or training, or home-educated children, receive the universal offer.
20. Undertake nurse-led immunisation programmes in schools and other venues following patient group directions.
21. Complete PDRs and return to work pro-forma as required to designated staff; ensure timely performance data.
22. Cover for named team leader as required.
Qualifications and experience
Essential — Qualifications: Registered Nurse; Community Specialist Practitioner in Public Health (School Nurse); First level degree; Mentorship Qualification. Experience: Post-registration experience; community/public health experience with children and families; safeguarding and policy implementation; multi-disciplinary/multi-agency working; knowledge of Health Child Programme; understanding of public health outcomes and inequalities.
Desirable — Family Planning/Sexual Health; Clinical supervision/preceptorship; experience managing teams; HR skills; experience with sickness, disciplinary, grievance and capability issues; experience with spreadsheets/databases.
