Main duties of the job
To work as partof a multi-disciplinary team to provide a high standard of specialistpalliative care to patients and their families. To lead the shift, ensuringthat high standards of care across the IPU are maintained.

Our InpatientUnit (IPU) has eight single, en-suite rooms where we care for patients who havebeen admitted for symptom management and/or end of life care.

Job responsibilities
Clinical
Tobe a safe practitioner and work within the NMC Code of professional standardsof practice and behaviour.
Provideholistic assessment of patients and families care needs, plan interventions
requiredand ensure those interventions are actioned and their effectiveness evaluated.
Tocommunicate effectively with patients and families, ensuring they are activelyinvolved with their own care planning.
Tosupport patients and families through grief and loss and the life changingconsequences of a palliative diagnosis.
Toliaise with appropriate members of the multi-professional team (internal andexternal to the organisation) in order to ensure co-ordination and continuityof care on discharge and a smooth transition between different care settings.
Tomaintain clear, accurate and legible patient documentation.
Towork across the 24-hour service, including weekends, nights and bank holidaysas required.
Education
Tosupervise and support junior colleagues, volunteers and students working on theIPU.
Toactively participate in ward meetings and ensure that accurate information isfed back to the appropriate members of the multi-disciplinary team.
Totake responsibility for own ongoing education and professional developmentplan.
Tosupport and participate in the education and development of colleagues.
Tobe involved in the training programme of professional visitors/learners whereappropriate.
Tomentor new staff members and students.
Management
Toact as a role model for all colleagues, providing encouragement and motivationto achieve goals for the service.
Totake charge of a shift, maintaining high standards of care escalating issues asrequired.
Totake responsibility for the care of their allocated patients.
Tomaintain a safe environment for those within the IPU.
Tobe aware of cost implications and make effective use of resources.
Toapply evidence-based practice in their own nursing care and to encouragecolleagues to do the same.
Towork within other clinical areas within the Hospice as required.
Toactively participate in multidisciplinary team meetings and ensure thataccurate information is shared with IPU team.
Quality
Toactively participate in, quality improvement, clinical audit, and research as ameans of monitoring and improving standards of care.
Totake responsibility for a link nurse role on the IPU, tissue viability,infection control etc.

Qualifications

1. First level Registered Nurse
2. Evidence of personal professional development

3. Qualification in specialist palliative care
