An exciting opportunity has arisen for Care Navigation Officers to join the Care Navigation Service.
You will work as part of a team of caring and compassionate individuals who are liaising with internal colleagues and external Care providers daily, using ICT systems and software and telephone communication to source packages of care for residents in need in Lancashire.
You must be highly organised, be able to juggle conflicting responsibilities, adhere to tight deadlines while working with accuracy and attention to detail in a fast paced and often challenging environment.
Development: No sourcing experience is needed. You will be provided with training and continue to be supported to enhance the skills you have to carry out the role.
We are looking to recruit a full-time temporary Officer. The successful applicant will be based in and working in the Preston office at County Hall Complex.
What we are looking for
* You will be responsible for sourcing care packages with external care providers for Lancashire residents using relevant IT systems and software and completing all associated administration tasks.
* You will have an allocation of work to manage.
* The role requires excellent communication skills orally and in writing with internal and external customers and colleagues at all levels using a variety of media such as telephone, electronic and other methods.
* Ability and knowledge of following processes and procedures and working within legal frameworks.
* The ability to adhere to strict timescales and escalate cases where necessary.
* Maintain timely, accurate and up to date electronic records and information for management and other professionals while maintaining data security.
* Work as part of a busy team and in collaboration with colleagues to ensure service targets are met.
* Work with limited supervision using creative thinking to solve problems.
* Develop excellent working relationships both internal and external.
* Participate in rotas to cover miscellaneous tasks such as allocating, dealing with team mailbox queries, and updating then sending out regular service information.
* Work closely with colleagues, managers, and other professionals to deliver an effective and high-quality service.

The service is open 364 days of the year (closed Christmas Day). Successful candidates will participate in a rota determined by the line manager to cover working hours between 8am – 6pm Monday to Friday, as well as weekend and bank holiday working between 9am – 5pm.
