What the job involves As a Leaving Care Worker, your role is to be a key point of contact and coordination for young people transitioning out of care. You will manage a caseload, working directly with youth and their networks to implement effective plans that support their independence. Key responsibilities include encouraging youth involvement in decision-making, coordinating with agencies, and fostering a supportive network to ensure positive outcomes in education, employment, and well-being. You will work flexibly, including evenings and weekends as needed, to provide guidance and crisis support, maintaining accurate records and working collaboratively within a multi-agency team.
