Health Care Assistant - Community Heart Failure Team

We are looking for an organised, enthusiastic and self‑motivated individual to provide Health Care Support to a small friendly team of Community Heart Failure Specialist Nurses in Dorset.
Experience of working in a health care setting is essential for this post. You will need to have excellent interpersonal and communication skills with an ability to use your own initiative as well as work effectively as part of a team. You must be flexible and have a positive approach to your work. The post holder will be required to communicate effectively with patients, family, carers and members of the public and staff.
You will cover a wide geographical area predominantly visiting people across West, Mid and North Dorset; however you will be part of the wider team of Community Heart Failure specialist nurses covering the whole of Dorset.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service may present a valid DBS certificate instead of requiring a new check.
Main duties of the job
The role will include, but is not limited to:
* Assisting the Community Heart Failure Specialist Nurses in the care planning, assessment, monitoring and review process of patients referred to the Community Heart Failure Service.
* Independently visiting patients in their own home to carry out a basic heart failure assessment and undertake simple clinical tasks such as weight, blood pressure, pulse and oxygen saturation monitoring, venipuncture sample collection, skin care inspection and prevention of pressure sores as delegated under the guidance of the Community Heart Failure Nurse. Other tasks may be requested when necessary such as recording ECG's, however if needed training and competency for this will be provided.
A car driver and owner is essential. You will be reimbursed for your travel expenses.
Knowledge of Systmone would be advantageous.

Knowledge
* Skills to manage occasional challenging situations
* Good organisational and decision‑making skills
Experience
* Primary/Community healthcare experience
* Previous experience of providing education and support for patients/carers.
* Previous experience with patients with a chronic health condition
Skills
* Demonstrable skills in written and spoken English, adequate to enable the post holder to carry out the role effectively
* Ability to work autonomously and demonstrate initiative.
* Ability to deal with a range of people at all levels, including patients, carers, health and social care colleagues and other professionals
* Ability to understand work related boundaries and refer matters of importance to a senior colleague
Qualifications
* NVQ / Diploma Level 2/3 in healthcare or willing to work towards
