Are you an experienced nurse looking to join an exceptional primary care team?
DMC Primary Care seeks a personable, motivated, and experienced Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team of primary care providers.
This full-time position is based in our Raymond and Dover, NH clinics.
DMC offers a fast-paced, supportive team environment and the opportunity to care for patients with a wide variety of medical conditions, from acute issues to wellness visits.
We aim to build long-term relationships with our patients and help improve their health and wellbeing.
The successful candidate will be physically present in our Dover and Raymond offices.

Responsibilities include:
* Performing nurse visits in the clinic, including INR checks, coumadin management, vaccine and medication administration, heart monitor placement, EKG, sleep device instruction, PFT, and managing triage calls.
* Handling calls from patients, families, care teams, and other entities, providing nursing judgment on medications, prescriptions, referrals, immunizations, lab results, symptoms, etc.
* Managing complex messages from providers and notifying patients of abnormal lab or imaging results requiring further workup.
* Serving as a liaison between providers, care teams, and patients regarding quality-of-care initiatives through daily communication.
* Participating in health education related to preventive care as needed.
Qualifications:
* Graduation from an accredited nursing school is required.
* Current State of New Hampshire Nurse/LPN license is required.
* Effective communication skills, both oral and written, are essential.
* At least two years of acute or ambulatory care experience required.
