Overview
We are seeking to appoint an enthusiastic, motivated and reliable Community Health Care Support Worker to deliver a range of quality care services to patients visited by the Care Community Team based at Chowley Oak Tattenhall. The team visits patients across the Rural Alliance Care Community.
Responsibilities
* Work in patients\' homes as part of a friendly team.
* Carry out tasks such as simple dressings, catheter care, personal care and recording blood sugars.
* Provide end-of-life care and support to prevent hospital admissions and facilitate discharge from hospital.
* Lone working is involved; travel across the Western Cheshire footprint is required and access to transport for work purposes is essential.
Contract and responsibilities specifics
This role is part time – 30 hours per week.
The Trust supports the Real Living Wage (RLW) hourly rate and will apply a pay supplement if the national A4C hourly rate falls below this.

Induction, training and application process
New starters commence on one fixed start date each month, aligned with the Prepare to Care (P2C) induction, which offers up to 3 weeks training.
