This role has a starting salary of between £42,239 - £45,135 per annum based on a 36 hour working week. We welcome both full-time and part-time applications.

We are excited to be hiring a Social Worker to join our fantastic Transition Team based at either Victoria Gate in Woking or Woodhatch Place in Reigate.

This is a hybrid role where, as a team, we split our time between collaborating with colleagues in the office for a minimum of two days per week, and working within the communities we serve, including home visits.

About the Role

The Transition Team are a specialist Adult Social Care team supporting young adults from aged 14 as they move into adulthood. We work closely with young people and their carers and families.

The Transition Social Worker plays a critical part in delivering Surrey's strategic priorities for young people with physical disabilities, learning disabilities and autism as they move into adulthood, translating vision into meaningful, person-centred outcomes.

Operating at the intersection of policy, practice, and partnership, you will drive improvements in service quality, early intervention, and integrated working, ensuring that individuals and their carers receive the right support at the right time.

In this role, you will have the opportunity to influence how services evolve, contributing to innovation and continuous improvement across both Adults' and Children's Social Care. By building effective relationships with colleagues, partners, and local communities, you will help shape more responsive, inclusive, and sustainable approaches that align with long-term organisational goals.

This role offers significant scope to make a tangible difference, enhancing the experiences of young people with physical disabilities, learning disabilities, and autism, as well as their families and carers, while reducing inequalities and ensuring resources are used effectively. Through professional leadership, insight, and a commitment to best practice and sound budget management, you will deliver measurable impact at both operational and strategic levels.

Your key responsibilities as a Social Worker within the Transition Team will include:

10. Managing and prioritising a case load
11. Legal court work
12. Working closely with partners
13. Completing Section 42 enquiries
14. Working on duty
15. Supporting the team with professional development
16. Supervising Senior Social Care Assistants to ensure a high-quality standard of service is provided

Your Application

In order to be considered for shortlisting, your application will clearly evidence the following skills and align with :

17. Professional qualification in Social Work, current registration with Social Work England and completion of an Assessed and Supported Year in Employment if more recently qualified
18. Knowledge and experience of current legislation, including the Care Act and the Mental Capacity Act and its application to practice in a social care setting
19. Knowledge of relevant guidance and person-centred strengths-based practice
20. Knowledge of, and experience in, carrying out safeguarding enquiries, carrying out assessments within the hospital and/ or community and working with a diverse group of individuals and partner agencies
21. Experience of supporting non-qualified or less experienced members of staff with their development
22. Commitment to your own professional development

Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK and access to a vehicle and be willing to travel across a wide geographical area.
