Support medicines reconciliation and medication reviews, including reviewing clinic letters and hospital discharge summaries. Assist with monitoring requirements and shared care protocols. Undertake patient-facing roles to promote effective medicines use. Support structured medication reviews (SMRs) and organise necessary monitoring.
Provide lifestyle advice and address public health and social care needs. Promote antimicrobial stewardship and reduce inappropriate prescribing. Implement efficient medicines optimisation processes and reduce medication waste. Ensure legal, safe, and secure handling of medicines, including EPS implementation.
Promote Electronic Repeat Dispensing (eRD) and online ordering. Train and support practice teams in prescription handling and streaming. Support national prescribing policies and local enhanced services. Contribute to QIPP, QoF, and other PCN priorities.
