We are looking for a compassionate and enthusiastic Acute Illness Nurse to join our friendly and supportive clinical team.
Our Acute Illness Nurse plays a critical role in the management and treatment of patients presenting with minor illnesses and acute conditions within a general practice setting. This role involves the assessment, diagnosis, and treatment of a wide range of acute illnesses, ensuring timely and effective care for patients. The Acute Illness Nurse works independently but is supported by GPs, nurse practitioners, and other healthcare professionals to provide high-quality, patient-centered care.
We are a well-established practice serving around 6,200 patients, with an excellent reputation for high-quality, personalised, patient-centred care. Continuity of care and the culture of traditional family medicine are strong values in the practice, and support our "cradle to grave" relationship with patients.
The post-holder will be responsible for assessing, diagnosing and treating patients with acute conditions. You will also provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.
We operate a full clinical triage appointments system, and our appointment system supports a range of communication channels including telephone consultations, e-consultation, and 15-minute face-to-face appointments.

Main duties of the job
You will have the ability to make professional autonomous decisions when consulting with patients with undiagnosed acute illness, and make assessment of their health care needs.
As an acute illness nurse you will be seeing patients independently in 15 minute Face-to-Face appointments.
Within patient consultations you will use highly-developed nursing knowledge and skills such as physical examination, applied pharmacology and evidence-based prescribing.
Maintain accurate and timely patient records using clinical IT systems (e.g. EMIS).
Work collaboratively within the practice nursing team, GPs, and other healthcare professionals

Job responsibilities
* To assist medical personnel in the care of practice patients to include treatment, preventative care, screening and patient education. The post will also include some administrative duties.
* To act as a focal point of communication between patients, doctors, nurses and other members of the primary health care team
* To maintain a safe and effective level of practice and to bring any deficits in training to the attention of the nursing team leader so these can be addressed.
* To act in a professional manner at all times and adhere to practice policies, protocols and guidelines.
* To work as an effective team player
* To provide continuing care to patients across the whole practice population.
* To provide health maintenance and health promotional services to the well, offering educational supportive setting for patients to explore their health problems.
Person Specification
Experience
* Minimum 2 years post-registration experience
* At least 1 year primary or community nursing experience
* Aware of accountability of own role and other roles in a nurse-led service
Qualifications
* Degree or equivalent in Nursing and on NMC register.
* Experience of independently dealing with day to day acute illness in primary care, community or in Accident and emergency.
* Ability to identify determinants of health in the local area.
* Knowledge of public health issues in the local area.
* Awareness of local and national health policy.
