About the Job:
The Bracebridge team is looking to recruit health care assistants for full and part-time hours.
The team provides fast-paced community services responding to planned calls.
The service is part of the wider admission avoidance offer. The post holder will need to be dynamic in their approach. Health care assistants visit individuals in their homes, clinics, or GP surgeries under the supervision of a registered professional. They work collaboratively with patients, carers, and other health and social care agencies to promote independence, support team roles, and contribute to service development.
The post requires shift work across a 7-day period to ensure continuous service. Many staff may have second jobs, or the evening service might be their sole role. We welcome all suitable applicants. As a Trust, we are flexible and seek a caring, adaptable professional to work across the city-wide service.

The successful applicant will:
* Possess an NVQ Level 3 in Health and Social Care or equivalent.
* Have experience dealing with the public and working in clinical or home environments.
* Show commitment to teamwork.
* Hold a current driving license and be able to travel independently across the BCHC area.
* Be capable of working independently and as part of a multidisciplinary team. Excellent communication skills and the ability to meet diverse needs are essential.
