Care UK is looking for a Care Team Leader to lead and supervise a shift, case manage residents, and coordinate care with families and healthcare professionals.
Responsibilities
* Lead and supervise the care team during shifts.
* Case‑manage residents and liaise with families, healthcare professionals, and other stakeholders to ensure that the best care plans are created and delivered.
* Administer and record medication.
* Carry out risk assessments.
* Work alongside care assistants to support residents with daily needs, ensuring residents remain as active and independent as possible.
* Collaborate with the leadership team to develop services.
* Maintain a fun, inclusive, and professional approach at all times.
Qualifications and Skills
* NVQ Level 3 in health and social care (desirable).
* Proven ability to lead and supervise others.
* Excellent organisational and IT skills.
* Experience or willingness to undertake training on administering medication.
* Strong understanding of care planning processes.
* Natural ability to help make the lives of others more fulfilling.
