Overview
Are you a qualified Physiotherapist wanting to further your rehabilitation, organizational and management skills, working with patients in their own homes? We are seeking an enthusiastic Band 6 Physiotherapist to join our Planned Care team in Watford community. The position offers experience of a mixed caseload of patients with various pathologies, including amputees, neurological/stroke rehabilitation, Parkinson's disease, falls/balance impairments, orthopaedics, long-term conditions and palliative/end of life care. The aim is to rehabilitate and maximise the patients' potential, to support and enable patients in a range of activities to promote independent living in their home environment. The team is well-established, covers a large area and population size, and the position requires assessment and treatment of physiotherapy interventions for patients in their own homes. Liaising with patients, family, carers and other MDT staff.
This is an excellent opportunity for a physiotherapist working in acute, bed-based rehab, or social services, who has completed core rotations in acute setting as a band 5, and would like to experience community working.

Responsibilities
* Provide assessment, treatment, and interventions to patients in their own home environment to support them to optimise their functional ability.
* Autonomous practitioner with close collaboration with physiotherapists, assistants and occupational therapists.
* Maintain close links with the community nurses, complex care and frailty team, GPs, palliative care specialists, hospices, social care, and other third-party organisations.
