Immediate Start Available Occupational Therapy Assistant - Rhondda Council Salary: £180 Per Day Hours: 37 Hours Working Schedule: Monday to Friday Contract Length: 3 Months Location: Rhondda Cynon Taf, CF401NY Job Details To assess the needs of people and carers with regards to the provision of moving and handling or other specialised transfer equipment to support the safe delivery of care packages to ensure that people can remain safe in their own home. Following assessment, write comprehensive moving and handling plans / risk assessments to inform Care and Support teams, community health professionals and domiciliary care providers (internal and external) of the requirements for the safe delivery of a care package. This will also inform the resultant Care and Support plan. To undertake person centred functional assessments and provide recommendations for the provision of equipment and low/medium level adaptations. Undertake an assessment of a person’s circumstances in relation to their difficulties accessing suitable parking to inform the annual Disabled Parking Bay Scheme allocation panel. SPECIFIC RESPONSIBILITY Assessment of people where the referral indicates that there are moving and handling, hoisting, transfer or management issues where the provision of equipment and completion of a moving and handling risk assessment and / or plan may be the solution. Assessment, recommendation and ordering of low/medium works of adaptation and/or related equipment. Providing information, advice and signposting to other services for people and carers including referral on to a Care and Support Team where necessary and appropriate. Undertake liaison with a range of health and social care professionals in relation to moving and handling cases. Provide advice and support for care managers / providers in relation to moving and handling or works of adaptation. Liaison with housing colleagues in respect of works of low/medium adaptation – RCT Housing Grants, Trivallis, Registered Social Landlords, Care and Repair etc. Carrying out evaluations of completed works of adaptation and outcome of equipment provision, and where necessary, refer to an Occupational Therapist for further specialist assessment e.g. major works of adaptation, complex moving and handling situations. Use WCCIS system to ensure accurate and timely recording of assessments, plans and case notes as per the Case Recording Policy. Provide the annual Disabled Person’ Parking Bay allocation panel with an assessment, resulting in a score, in regarding a person’s difficulties accessing suitable parking close to their home. Assessment Using WCCIS and referral information/risk assessments received from care providers to evaluate potential risks and support needs. Arranging appointments for assessment visits Adopting a person centred, creative and holistic approach whilst exploring options. Seeking out solutions with people and carers that support their independence, inform choice, and agree personal outcomes to be achieved. Attendance as required at meetings in relation to moving and handling issues to provide information and advice. Identification and evaluation of risks in relation to people, carers and providers via the assessment process. Provide advice regarding the private purchase of equipment Arranging for the provision of moving and handling equipment Provision of moving and handling plans in a timely way to providers to enable care and support package to commence. Provision of advice and support to colleagues and teams in relation to moving and handling issues. Arrange for low/medium works of adaptation to be carried out where necessary to ensure a functionally suitable and safe environment for the person, which promotes and/or maintains their independence. Recognise when to seek advice and support from Senior Occupational Therapist / Team Manager. Ensuring that people and carers are aware of the Comments and Complaint process and the availability of advocacy services. Ensure that assessments regarding a person’s difficulties accessing suitable parking close to their home for the Disabled Person’s Parking Bay scheme, when reviewed by the panel, evidence a consistency in the scoring applied. Provision of service and review Engaging directly with people, their carers and all relevant agencies involved to review moving and handling plans, including the suitability of equipment provided, and amend plans/recommendations where necessary. Review and evaluate the provision of equipment and low/medium level adaptations in relation to whether the person’s outcomes are being achieved. Managing workload and administration. Use WCCIS to manage workload from the ACE Waiting list, to include self allocation of work, as overseen by the supervising Senior Occupational Therapist/Team Manager. Ensure the accurate and timely recording of electronic assessments, plans and case notes as per the Case Recording Policy. Timely administration of all aspects of the caseload e.g. onward referrals, completion of requests for works of adaption, ordering equipment, production of moving and handling plans. Preparing and engaging in supervision with line manager. Establishing and maintaining good working relationships with colleagues. Identifying and addressing areas for development and training. Agreeing and adhering to any action plans as a result of performance evaluation. Ensuring that agile working devices are kept updated and in good working order. Contribute operational knowledge to help shape services and operational systems, implement projects and new legislation and polices, with the aim of achieving better outcomes for service users. Maintaining competence. Must by trained in Moving and Handling Passport and undertake refresher training on an annual basis. Liaise with the Moving and Handling coordinators in the training department and keep informed of developments in relation to specific areas of complexity within moving and handling services, for instance, management of bariatrics clients, single carer ethos, management of clients with behavioural or long term mental health conditions in relation to developing a moving and handling package that will meet specific needs. Must develop and maintain extensive knowledge of moving and handling equipment and cost effective options. Must participate in a minimum of 2 per observation of practice per year, or as agreed with the line manager. Continuing development is essential for the OTA to ensure the quality and standards for the service are maintained and improved where necessary. Must be willing to undertake additional specialist workshops on moving and handling techniques and developments. OTA staff will be expected to undertake agreed and recognised QCF qualifications as part of their continuing development. OTA staff will have the opportunity to attend in house and external training which will support and develop practice or which is a statutory/legal/health and safety requirement to enable staff to undertake specific assessments in the course of their duties, Individual training records and development information will be maintained by individual staff and used in conjunction with supervision records to identify areas of need.
