Aberdeen City Out of Hours Nursing Band 6 Senior Community Nurse position within the Aberdeen City HSCP Evening and Night Community Nursing Unscheduled Care Team. The Senior Community Nurse works with and supports Band 5 and Band 3 colleagues delivering a person-centred clinical nursing service that prioritises end of life care and symptom control for patients in their own homes and plays a vital role in supporting admission avoidance. The role includes triage and management of unscheduled nursing referrals from GMED, NHS 24, Hospital at Home and other professionals, and provides support for the District Nurse with team management responsibilities.
Responsibilities
* Deliver a person-centred clinical nursing service prioritising end of life care and symptom control for patients in their own homes.
* Triaging and management of unscheduled nursing referrals from GMED, NHS 24, Hospital at Home and other professionals.
* Provide support for the District Nurse with team management responsibilities.
* Collaborate with Band 5 and Band 3 colleagues to co-ordinate care and ensure admission avoidance where possible.
Qualifications and experience
* Graduate Diploma in Integrated Community Nursing, or a relevant Post Graduate Certificate.
* Evidence of current clinical experience in a Community Nursing role at Band 6, including triaging of referrals, workload planning, palliative and end of life care.
Hours, location and requirements
* Hours: 1800-0000, 2 or 3 shifts within an 8 day rota.
* Car owner or access to a car/driver and current UK driving licence is required. Car must be LEZ compliant.
* Location: Aberdeen Community Health and Care Village, White Zone, Frederick Street, Aberdeen AB24 5HY.
