Antimicrobial and HIV Specialist Pharmacist

Provide specialist pharmacy advice and training in the areas of HIV and antibiotic stewardship to consultant & junior medical staff, nursing staff, pharmacy staff, other healthcare professionals, and patients/carers.
Assist in the provision of regular structured feedback to the HIV team regarding the safe and effective use of medicines and drug expenditure to facilitate service improvement.
Undertake and contribute to clinical audit and quality improvement within Pharmacy and the Specialties.
Main duties of the job
Providing core clinical pharmacy service to HIV patients and inpatients receiving antibiotics in line with the Trusts clinical pharmacy standards.
Support the Acute Care at Home pharmacist ward rounds, when required, to identify patients for home IV antibiotic management via the Acute Care at Home service.
Lead antibiotic stewardship ward rounds to optimise inpatient antibiotic prescribing.
Ensuring through clinical screening that high quality prescribing is provided on all reviewed patients.
Ensuring that pharmaceutical knowledge and skills are applied through proper medication reconciliation on admission, optimisation during their admission and input to the discharge process when appropriate.
Actively providing medicines information and medication counselling to health care professionals and patients and carers.
Working with medical staff to formulate medicine policies and guidelines that promote rational & economical prescribing. Monitoring practice to ensure policies and guidelines are adhered to.
Assist in the development and implementation of systems within the Trust, to ensure safe, economic and clinically effective use of antimicrobials in all patients in line with local and national initiatives.

Qualifications
* Pharmacy Degree
* GPhC Registration
* Clinical diploma or equivalent or working towards
* Non-medical prescriber
* RPS membership
Experience
* Significant post registration covering a range of specialities
* Sufficient experience as a clinical pharmacist
* Clinical supervision or training of junior staff
* Experience of deputising for a more senior clinical pharmacist
