CYPMHS West Herts is seeking an inspirational Community Team Leader who is a highly skilled, motivated practitioner with the appropriate professional qualification and experience to work in a multi-disciplinary team. The position is full-time, 37.5 hours per week, and permanent.
Key Responsibilities:
1. Work across two sites: St Albans and Hemel Hempstead.
2. Lead a team working with children and young people presenting with emotional and behavioural needs, including complex trauma and attachment difficulties.
3. Have an in-depth understanding of neurodiversity and be able to identify reasonable adjustments to support service users.
4. Work in line with CYP-IAPT principles and practices.
5. Lead and ensure high-quality care delivery, collaborating with the CYPMHS Operational Manager and Clinical Team Leader.
6. Manage key performance indicators and throughput based on CAP, a service transformation model used in CYPMHS.
7. Act as care coordinator when appropriate, ensuring care plans are in place, and monitor multidisciplinary interventions.
8. Work sensitively with families from diverse racial, cultural, and religious backgrounds, including various family structures.
9. Maintain a professional profile in accordance with Post Registration standards.
