We are looking for warm, caring individuals to provide person-centred support to service users with complex mental health needs in the Northenden area. Your role will include:
1. Maximizing service user outcomes by promoting independence and social inclusion
2. Encouraging service users to be involved in decisions regarding their care and lives
3. Providing personal care, practical and emotional support to adults with mental health needs
4. Collaborating with service users, their families, and involved professionals to provide a consistent and coordinated service
5. Developing and maintaining health action plans
6. Maximizing and developing communication plans for service users
The Support Living Coordinator role will be to lead the team in providing individualised, person-centred support to people with mental health needs living in their own homes. The goal is to enable them to enjoy a fulfilling and valued life, participate in the community, and develop their abilities as fully as possible.
You will have good leadership skills, a warm, caring nature, and a commitment to making a positive difference in people’s lives. Flexibility is required, including evenings and weekends, to meet the needs of the service.
