Education / Qualification
Essential criteria
1. Evidence of extensive experience and knowledge in complex discharge planning. Ability to teach and assess learning needs of junior staff
2. Evidence of working across different agencies, recognising the contribution made by each agency
3. Educated to diploma level or relevant professional development
4. Working experience in both Health/Social Care setting
Desirable criteria
5. IT Certificate in advanced Excel/Access and Word
Knowledge & Experience
Essential criteria
6. Experience of discharge planning within the acute setting
7. Experience of working with older people and their specific need
8. Proven experience at working across directorate
9. Experience of managing complex situations and suggesting/discussing solutions which result in positive outcomes.
Desirable criteria
10. Understanding of Discharge to Assess Pathways
11. Experience of working alongside community partners
12. Understanding of the principles of patient flow or similar operational role
Skills
Essential criteria
13. Excellent organisational skills,communication and documentation skills
14. Excellent interpersonal skill and risk management
15. Well- developed negotiating and motivating skills.
16. Able to deal confidently and professionally with all members of the MDT, internally and externally to the organisation, including patients and their families.
17. Able to collate and present written, electronic and verbal information in a concise format from complex multi-factorial sources.
