Job overview: An exciting opportunity has arisen for a Band 5 Community Nurse to join the Leeds Intensive Support Team. This role is designed to enable a Band 5 nurse to pursue career development by gaining skills and confidence to develop into a Band 6 nurse while in the team. The successful candidate will be supported through managerial and clinical supervision to explore a work plan and competencies essential for a Band 6 nurse.
The Intensive Support Team for people with learning disabilities works with individuals requiring additional support due to mental health or behavioral needs, often experiencing trauma, personality disorders, or mental health challenges. The team aims to keep individuals out of hospital, ensuring safety, empowerment, and skill development.
Main duties of the job:
* Work as a valued member of the multidisciplinary team.
* Collaborate with professionals working with individuals on the IST caseload.
* Use evidence-based and client-centered principles.
* Assess, plan, and implement care to meet behavioral and mental health needs, aiming to prevent hospital admissions.
* Lead in developing and maintaining nursing care to promote health and wellbeing.
* Maintain links with professional structures and support professional development.
* Stay updated with clinical practice through MDT meetings, supervision, and forums.
