Responsibilities
The goal of this job is to lower the number of unnecessary hospital stays, help carers break down, and encourage independence by evaluating people in their own homes and helping them live as independently as possible.
* Assist the manager in providing a full range of services, ensuring vulnerable persons are safe.
* Ensure adults and their carers can receive timely help during situations such as falls or carer emergencies.
* Determine eligibility for services, develop care plans, and maintain client wellbeing.
* Collaborate with health professionals from different agencies in an integrated service.
Qualifications
* Registered with Social Work England.
* Strong communication and interpersonal skills.
* Knowledge of relevant legislation and frameworks for adult social care.
* Driven, structured, energetic, and creative.
