Cardonald Medical Centre

NHS Greater Glasgow and Clyde (NHSGGC) is one of the largest healthcare systems in the United Kingdom, employing approximately 40,000 staff across a wide range of clinical and non-clinical professions and roles. We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.

The role:

The GP Walk in Centre is based at Cardonald Health centre in South Glasgow and will provide urgent primary care services to the local population. The service aims is to improve access, reduce pressure on GP practices, and minimise inappropriate Emergency Department attendances by offering rapid access to same-day assessment for patients unable to secure a GP appointment.

We are looking for motivated, enthusiastic Advanced Nurse Practitioners who have experience of working in primary care at an advanced level. Experience in Acute Care would be an additional advantage.

The Advanced Practitioner will practice autonomously in the assessment, diagnosis and appropriate management of patient presenting with undifferentiated and undiagnosed problems. They will be supported by a team of GPs and nursing staff on site to deliver urgent care and appropriate interventions as required.

You need to have a live NMC UK pin in order to be considered for this post.

Duration, Location, and Working Pattern:

1. Location based Cardonald Health Centre
2. Fixed-term Contract for 12 months
3. Part-time position (24.5 hours per week & 16.5 hours per week)
4. Shift pattern for this post are:

24.5 hours per week - 3 days a week

16.5 hours per week - 2 days a week

5. Please note the salary is Pro Rata for part time hours.

Key responsibilities:

6. Assess each patients needs and treat accordingly utilising evidence based practice. This will involve utilising highly developed clinical knowledge and advanced decision making skills to facilitate a full analysis and interpretation of the patients history, presenting symptoms and results of clinical examination to ensure an accurate diagnosis and subsequent management plan.
7. Implement advanced clinical care, incorporating advanced clinical skills within agreed protocols and guidelines, and within this make decisions to admit, refer and discharge.
8. Apply high level decision-making and assessment skills to formulate appropriate differential diagnoses based on synthesis of clinical findings.
9. Possess the freedom and authority to request, where indicated using judgement and clinical reasoning, appropriate diagnostic tests / investigations based on differential diagnoses.
10. Interpret and analyse previously ordered results of tests/investigations and work collaboratively with other healthcare professionals when needed.
11. Adopt a multi-agency approach to provide person-centred care throughout the continuum of care.

Knowledge, training, qualifications and/or experience required to do the job:

Essential:

13. Registered Nurse on the NMC Register
14. Evidence of completion of Masters level education in Advanced Practice
15. Non-medical Prescriber V300
16. Proven experience of working in primary care at an advanced level
17. Professional registration with NMC/HCPC
18. Be recorded as a Non-medical Prescriber (V300) with the NMC
19. An appropriate Masters level qualification in Advanced Practice which has enabled competence in:

- Clinical assessment

- Clinical reasoning, judgement and diagnostic decision making

- Anatomy and pathophysiology

- Non medical prescribing- V300

- Leading, delivering and evaluating care

- Practice learning/transferable work based learning

20. Significant post-registration experience within a healthcare setting, some of which must be in acute care
21. An Advanced Nurse Practitioner with Primary Care experience
22. A commitment to lifelong learning and demonstrates evidence of continuing professional development
23. Basic Life Support (BLS) essential

Desirable:

24. Can demonstrate experience in managing staff, leading a team, providing education and training and establishing a supportive, learning environment
25. Successful track record in working in multi-disciplinary team settings and promoting collaborative approaches to delivering excellent patient care
26. Immediate Life Support
