Job Overview
You will facilitate the smooth running of the patient journey by ensuring that coordinated administrative processes are in place. Managing a list of patients and providing full administrative support to the Local Cancer Multidisciplinary Team Meetings. Proactive monitoring and tracking of the cancer patients ensuring that their journey is processed in a timely and efficient manner, in line with Cancer Waiting Times. Working closely with other hospitals to ensure appointments and investigations are arranged in a timely fashion.
Responsibilities
* Attend weekly MDT meetings and PTL tracking meetings and expedite the tracking of cancer patients.
* Produce a list of patients to be discussed at the weekly MDT, including all necessary clinical information such as radiology and pathology results.
* Ensure diagnostic investigations are carried out in a timely manner and the results are made available as soon as possible to clinicians.
* Track and guide (virtually) the patient through the hospital system, ensuring there are no system delays that would prevent the patient achieving the cancer waiting time targets.
* Collect clinical information at the MDT such as staging information and pathology.
* Identify and resolve delays in the patient pathway; where this is not possible, discuss and agree ways forward with the Divisional Manager and Cancer Performance Manager.
* Ensure cancer waiting time targets are fully integrated into MDT meetings and clinicians have a full understanding of what is required.
* Input all data relating to the patient journey onto SCR (Somerset Cancer Register) and highlight any regular system bottlenecks, contributing ideas to resolve them.
* Work closely with the Cancer Performance Manager to continually improve the patient journey.
Candidate Profile
The successful candidate must have excellent attention to detail and good organisational skills. They must be confident in speaking with staff members from a range of multidisciplinary teams and support the coordination of meetings, including live typing of clinical discussions.

Additional Information
