Working as part of HASP PCN, the Care Coordinatorswill take the lead on specific initiatives within the PCN, namely enhancedhealth in care homes, and the extended access initiatives. The roles are highly dynamic however andduties may change in response to new initiatives or changing priorities withinthe PCN.
The successfulcandidates will act as first contacts for their specified initiatives and willlead on the implementation of the specific goals attached to eachinitiative. Key aspects of the rolesinclude communicating with a wide range of organisations and/or individuals,planning service delivery and ensuring that the services provided by the PCNrun efficiently and effectively.
Main duties of the job
Proactivelyidentify and work with a cohort of people to support their personalised carerequirements, using the available decision support aids
Bringtogether a persons identified care and support needs and explore their optionsto meet these into a single personalised care and support plan, in line withperson-centred service plan (PCSP) best practice
Help peopleto manage their need, answering their queries and supporting them to makeappointments
Ensure thatpeople have good quality information to help them make choices about their care
Supportpeople to understand their level of knowledge, skills and confidence when engaging with their health and wellbeing, includingthrough use of the patient activation measure
Assist peopleto access self-management education courses, peer support or interventions thatsupport them in their health and wellbeing
Providecoordination and navigation for people and their carers across health and careservices, alongside working closely with social prescribing link workers,health and wellbeing coaches and other primary care roles
Support the coordination and delivery of MDTs within PCNs

Job responsibilities
The post-holder will be expected to provide operationalsupport and coordination for the delivery of the Primary Care Networks servicesand projects, ensuring that services are well planned, delivered safely andeffectively and continually evaluated. The postholder will work with theGoverning Board, Clinical Director, Strategic Manager and PCN Member practicesand other bodies as required for the delivery of specific objectives (dependingon the initiative itself) to improve the quality and experience of the serviceswe provide to our patients.
